Introduction: Aid for Decision-making in Occupation Choice (ADOC) is an iPad-based assessment tool developed in Japan in 2011 to support shared decision-making in occupational goal setting, including for clients with communication and cognitive impairments. Objectives: This study aims to develop the Korean version of ADOC (K-ADOC) by evaluating the cultural relevance of the Japanese version’s domains and items through expert review, image analysis, and translation. Methods: Following World Health Organization guidelines, a six-step translation and validation process was conducted. Three occupational therapy professors reviewed the Korean translation, while one bilingual professor assessed accuracy. A comprehension survey was administered to 15 clinicians. Back-translation was performed by a Japanese-fluent expert unfamiliar with the original ADOC. Results: Revisions were required in mobility, sports/physical activities, and hobbies based on image and text evaluations. Content validity analysis by 15 clinical experts showed that self-care, home life (1 item), work/education (2 items), and interpersonal relationships (1 item) had content validity ratio (CVR) values below 0.49, requiring further modifications. Conclusion: The translated K-ADOC reflects Korean cultural context through expert evaluation and validation. While content validity was assessed, further revisions in key domains are needed. Future research should empirically validate the revised version for clinical application.
