Effects of rehabilitation based on Process Analysis of Daily Activity in community-dwelling patients with mild to moderate Alzheimer’s disease: A non-randomized controlled trial

Introduction: Rehabilitation for ADL/IADL independent utilizing residual ability are important viewpoint in community-dwelling patients with Alzheimer's disease to continue living in the community.

Objectives: To clarify the effect of rehabilitation based on Process Analysis of Daily Activity in community-dwelling patients Alzheimer's disease (AD) over 3-month period.

Method: Participants were community-dwelling older adults with mild-moderate AD, excluding those with severe physical impairment, and were recruited from outpatient rehabilitation etc.  35 participants in intervention group (25 women, 79.34 years) and 28 participants in control group (16 women 79.39 years) were included in analysis. Primary outcomes were Process Analysis of Daily Activity for Dementia (PADA-D) and satisfaction and performance with targeted ADL (10 point). In the intervention group, goal-directed ADL interventions were performed after clarifying PADA-D's impaired and residual processes. OTR visited patient's home in the intervention group once a week for 40 minutes for 3 months. The control group received only other home services. A University Hospital Ethics Committee (No. 190024) approved this study.

Results: Two-way ANOVA showed significant interaction total score (F=4.82), IADL score (F=4.12) in PADA-D and only laundry (F=6.24) among each ADLs between two groups. But, cognitive function and BPSD were no interaction. Landry in PADA-D consisted of 5 processes, "Operate the dryer", "Take in and fold the laundry" improved significantly compered to baseline. Satisfaction and performance for goals improved significantly.

Conclusion: IADLs performance and satisfaction for goal improved without changed cognitive function. We suggested that analyze process of targeted IADL and rehabilitation utilizing residual ability are important issues for community-dwelling patients with dementia.
