Shaping the future of occupational therapy: Telehealth measurement with the PACE framework

Introduction / Rationale: The use of telehealth to deliver occupational therapy (OT) services rapidly expanded because of COVID-19. The pandemic provided an opportunity to collect and analyze data related to the implementation of telehealth. However, there are limited systematic approaches to evaluation and measurement of telehealth delivery in OT. The goal of this presentation is to introduce a framework for telehealth measurement in OT research, practice, and policy.

Objectives: We developed the PACE Framework for measurement and evaluation of OT services delivered through telehealth. Drawing from existing frameworks of telehealth measurement, we proposed four domains with measurable sub-domains, and examples of outcome measures in OT: (1) Population and Health Outcomes; (2) Access for All Clients; (3) Costs and Cost Effectiveness; and (4) Experiences of Clients and Providers.

Methods / Approach: Following the American Occupational Therapy Foundation 2020 Planning Grant Collective, we conceptualized the PACE Framework and solicited expert reviews from key stakeholders. The PACE Framework was modified and finalized based on their feedback. The PACE Framework has continued to evolve and be refined to maximize usability.

Results / Practice Implications: Expert reviewers highly rated the utility, clarity, implementation, distinctness, and content of the PACE Framework. With this framework, OT practitioners can collaborate across settings and enhance the distinct value of OT delivered through telehealth. Using the PACE Framework, our profession can continue to build evidence that can be shared with various stakeholders.

Conclusion / Contribution to the occupational therapy profession: The PACE Framework provides researchers, practitioners, and policymakers with guidance to support meaningful data collection to illustrate the distinct value of OT delivered through telehealth across populations and practice settings.
