4M OT-AgeWELL: A scalable, innovative home modification model for safe aging in place across diverse global communities

Rationale: As the global population of older adults continues to rise, an innovative approach to home modifications is essential to support aging in place. By 2050, the number of older adults will double, with the majority living in low- and middle-income countries. Occupational therapy practitioners (OTPs) play a critical role in reducing fall risk, managing chronic conditions, and supporting cognitive health. However, there is limited evidence on the impact of low-cost, high-impact home modifications on health outcomes.

Objectives: (1) Introduce 4M OT-AgeWELL, a forward-thinking, OTP-led home-modification intervention grounded in the 4M Framework of Age-Friendly Health Systems. (2) Demonstrate the feasibility of 4M OT-AgeWELL in addressing home safety needs for older adults at increased risk of falls and hospitalizations.

Approach: 4M OT-AgeWELL reimagines home modifications through a structured, client-centered model targeting Mentation, Mobility, Medication, and What Matters. Led by an OTP, an interprofessional team—including a social worker and contractor—delivered the program through four home visits. Eligible participants were community-dwelling adults (62+) in urban environments. Sixty participants completed the program.

Practice Implications: 4M OT-AgeWELL is a scalable, innovative solution that fosters independence and well-being among older adults. Its success was reflected in strong participant engagement, with many referrals coming from previous program beneficiaries.

Significance to Occupational Therapy: 4M OT-AgeWELL embodies the future of occupational therapy by leveraging a structured, interprofessional approach to home modifications. By promoting safety, autonomy, and collaboration, this model inspires change and advances innovative solutions for aging in place.
