ProMOTE-ing adherence: An effective approach to aging in place

Rationale: Older adults wish to age in place and participate in their communities. However, adherence to occupational therapy (OT) recommendations, which is crucial for maintaining independence and safety, remains a challenge.

Objectives: To enhance the safety and independence of older adults aging in place by improving adherence to OT recommendations.

Methods: The ProMOTE intervention was developed using a comprehensive roadmap based on the Medical Research Council (MRC) framework, supplemented with elements from the Intervention Mapping approach. The Template for Intervention Description and Replication (TIDieR) checklist was used to detail the intervention. The content was scientifically substantiated through a meta-analysis and two qualitative studies, focusing on the needs and perspectives of older adults and their caregivers. This methodical triangulation led to the detection of the components that increase adherence.

Results: ProMOTE is a six-step OT intervention designed to maximize adherence. It includes evidence-based components essential for practical implementation in OT practice, emphasizing the importance of meaningful activities, health literacy, and interprofessional collaboration. The intervention addresses barriers to adherence by emphasizing strategies such as shared decision-making, personalized goal setting, the therapeutic use of self, timely intervention, and continuous monitoring.

Conclusion: By integrating scientific evidence and adapting to contextual needs, the ProMOTE intervention enhances the effectiveness of standard OT practices. It supports older adults in safely aging in place and maintaining social participation, thereby improving their overall quality of life and reducing the burden on caregivers.

A protocol is currently being developed for a randomized controlled trial to evaluate the effectiveness of ProMOTE.
